Steven, I think you are probably close to diagnosing. Maybe ADHD or some other constellation of issues. I was regaled with a similar "conversation" yesterday in a checkout line. As alluded to near end of it, sure does look to be a hard nutshell to keep up in. Regarding the latter post, can you still get away with making fun of short people..even in a drawing format?

Another way to look at it (direcor's POV) is human error is often more easily corrected for than an erratic piece of gear. I'm sure a lot of how this goes down depends on the context and tone of the moment.

Interesting use of the SVHS, Jeff. Years ago, I had a similar save on a multicam switching job I directed. It was a truck situation and when I began digitizing the Betacams, learned audio wasn't routed properly at all. In some cases, a slow speed but "H-Fi" VHS for quad split TC window logging wound up having the best audio! If a missed roll can happen in a narrative situation, have to wonder how often this happens in a chaotic reality/doco bag mix situation. I assume I'm missing a drawback but.. Wouldn't it make sense to have an established record triggering protocol in pro dual system situations? I mean beyond following time-code start..which won't help in continuous TOD use. Are you sure it uses P48? I understood it to turn itself on and off by its presence...saving internal battery when camera is shut off. "Switches on/off by P48 Phantom power"

The amount of info available via jwsound+____ on Google is incredible. FB may be good for speed in certain cases..but the resulting info seems a harder to find later.
