DPs who change camera settings in mid-stream, FCP editors who can't decide if a show is DF or NDF (remember the 'automatic' pull downs?), dub techs back when who thought a mono pilottone tape could be read and dubbed using just one of the 'stereo' tracks ('but it sounds mono'), even today's divergent a/v paths in a station and unpredictable pix processing in the home... ... and don't forget producers who sit in the back of a large theater and complain that dialog is late...
