how much delay between sound and video before it gets distracting

2 frames not too bad, a good dialog editor can see about a 1/4 frame out but ( my theory ) is most people won't see this but will still feel like something's not right

 - depends if you're mixing to the picture somewhat - im getting a feed that's 11 frames from vid split or 5.5 frames delay if I use the Qtake stream and it's a PITA

IMO 2 frames out would be the max - that is already 1/12 second or 83 ms (at 24 fps), and would be noticed by some people as "not quite right", as Joel points out above.

There is a new system from Teradek announced at NAB called SERV (Pro). Claims 4 frames. On the Teradek website even 2. 

Since it's not on the market yet we don't know if more viewers per unit will have an effect on the delay.

One frame out of sync is noticeable. Two or more frames out is distracting and unacceptable. It is a said state that we have become so accustomed to seeing sound and picture several frames or more out of sync, we have become numb to seeing lips out of sync with words, just like so many have become numb to out-of-perspective sound of body mics. While there might be reasons to have to deal with out-of-perspective sound, there is no excuse for living with out of sync sound.
