ProSound
I bought a yearly subscription for $29.00 a year it is great www.pdffiller.com It allows you to fill out any PDF form and sign it via your mouse then you can either print, email or fax the PDF. It also saves all forms online. I have been using it to fill out production paper work and time cards that I use to have to print and fax or print and scan. It has saved me a ton of time and paper so check it out.
ao
I use pdfpen: http://smilesoftware.com/PDFpen/ $59. download and handy for altering a sound report after you have made it a pdf. It can also store and scale your signature for forms requiring that and there is a demo available to play with. I do like notion of storing a filled out form online, in case you need it, which would be PDF Filler's trump card. ao
ProSound
I looked at PDF Pen but as you said I like the storing them online and the fact the program allows you to fax the forms as well if needed.

Daniel McIntosh
In the Preview application PDFs can be filled in using the Annotate tools. A "signature" can be created using the handwriting font. This works quite reliably, and has been improved with OS 10.6. Of course the occasional locked PDF will not allow any annotations.
